Delayed sleep-phase syndrome (DSPS), also known as delayed sleep-phase disorder (DSPD) or delayed sleep-phase type (DSPT), is a circadian rhythm sleep disorder, a chronic disorder of the timing of sleep, peak period of alertness, core body temperature, hormonal and other daily rhythms relative to societal norms. People with DSPS tend to fall asleep some hours after midnight and have difficulty waking up in the morning.

Often, people with the disorder report that they cannot sleep until early morning, but fall asleep at about the same time every “night”

Unless they have another sleep disorder such as sleep apnea in addition to DSPS, patients can sleep well and have a normal need for sleep

Therefore, they find it very difficult to wake up in time for a typical school or work day

If, however, they are allowed to follow their own schedules, e.g. sleeping from 4 a.m. to noon, they sleep soundly, awaken spontaneously, and do not experience excessive daytime sleepiness

The syndrome usually develops in early childhood or adolescence, and sometimes disappears in adolescence or early adulthood. Depending on the severity, it can be to a greater or lesser degree treatable. Prevalence among adults, equally distributed among women and men, is approximately 0.15% or three in 2000

DSPS was first formally described in 1981 by Dr. Elliot D. Weitzman and others at Montefiore Medical Center. It is responsible for 7–10% of patient complaints of chronic insomnia. However, as few doctors are aware of its existence, it often goes untreated or is treated inappropriately. DSPS is frequently misdiagnosed as primary insomnia or as a psychiatric condition. (By Wikipedia)

This is the most common circadian-rhythm sleep disorder that results in insomnia and daytime sleepiness, or somnolence.

A short circuit between a person’s biological clock and the 24-hour day causes this sleep disorder

It is commonly found in those with mild or major depression. In addition, certain medications used to treat bipolar disorder may disrupt the sleep-wake cycle

It often recommend chronotherapy to patients

This therapy, an attempt to move bedtime and rising time later and later each day until both times reach the desired goal, is often used to adjust delayed sleep phase syndrome

To adjust the delayed sleep phase problem, sleep specialists might also use bright light therapy or the natural hormone melatonin, particularly in depressed patients.

Hypnic headaches mostly strike the elderly people, age 50 and over.

Hypnic headaches is divided into the following types:

moderate

throbbing

bilateral

unilateral

Unilateral hypnic headache

This type of hipnic headache wakes the person once or many times during night

This headache starts a few hours after sleep begins and can last from 15 min to 3 hors or longer

There is normally no nausea, photophobia, phonophobia or autonomic symptoms associated with the headache

Hypnic headache commonly occur at the same time every night possibly linking the headaches with circadian rhythm, but polysomnography has recently suggests that hypnic headaches may be linked to REM sleep

Treatment

Lithium carbonate 200-600 mg at bedtime is an effect treatment for most patients.

For those people who can not tolerate Lithium, Verapamil, indomethacin or methylsergilide may be tried

Two patients have also responded to flunarizine 5 mg

It has also been shown that 1-2 cups of coffee ortrikes 1-2 mg of caffeine before bed can prevent hypnic headaches.

How hypnic headache is diagnosed?

Hypnic headache syndrome s diagnosed when it s at least 15 times per month for at least one month

Differential diagnosis of hypnic headaches in the elderly people are also identified by one or a few of the following causes:

drug withdrawal

temporal arteritis

sleep apnea

oxygen desaturated

pheochomatoma

intracranial causes

primary and secondary neoplasms

communicating hydrocephalus

subdural hematoma

vascular lesions

migraines

cluster headaches

chronic paroxysmal hemicrania.

All other causes must be ruled out before the diagnosis of hypnic headache can be made. (Wikipedia)

The disease is endemic in some regions of Sub-Saharan Africa, covering about 36 countries and 60 million people. It is estimated that 50,000 to 70,000 people are currently infected, the number having declined somewhat in recent years. Three major epidemics have occurred in recent history, one lasting from 1896–1906 and the other two in 1920 and 1970. In 2008 there was an epidemic in Uganda.

There are two types of Sleeping Sikness illness:

Developed by the Drugs for Neglected Diseases Initiative (DNDi) and partners, NECT comprises the administration of oral nifurtimox, and eflornithine injections for 10 days.

In medical terminology, an addiction is a chronic neurobiologic disorder that has genetic, psychosocial, and environmental dimensions and is characterized by one of the following:

The continued use of a substance despite its detrimental effects

Impaired control over the use of a drug (compulsive behavior)

Preoccupation with a drug’s use for non-therapeutic purposes (i.e. craving the drug)

Addiction is often accompanied by the presence of deviant behaviors (for instance stealing money and forging prescriptions) that are used to obtain a drug. (Wikipedia)

31-August 09

Q: Is anyone else addicted to sleeping? I sleep all the time and although some people say it’s normal for a teenager, my mom gets really mad all the time and yells at me a lot for it. I tell her I can’t help it but she think I’m just being really lazy. Also sometimes I just don’t feel like doing anything or putting something off until later. And sleeping is so peaceful.

A1: From Jim Bow

Well sleeping can be so comforting as well. It is not abnormal in itself, but what you need to check out is that you are not a little depressed or something. My daughters went through a phase of spending more time in bed than I thought healthy, but now they are both married it all different.Do have a physical check up just to be on the safe side. We all get moods when we feel it is better to stay in bed and do nothing. Best Wishes.

A2: Well, I do think it is normal for teenagers to like to sleep a lot. When I was a teenager, everyone I knew would sleep late on the weekends. Sometimes even until three or four in the afternoon, though I myself never slept past noon. My family got mad and thought us teenagers were being lazy, too. They thought everyone should be up by eight at the latest. So, I’d say that whether the issue is a cause for concern depends on what’s happening. What does your mom expect? How long and how much are you sleeping? If you do spend most of your free time in bed, then perhaps you should check things out.

A3: I sleep in but most i get is 10 hours sleep. the least is 6 hours sleep and i notice if i sleep too much and do too little then i get tired and i stay tired and sleep more that is why i try do more in the day otherwise i would sleep too much maybe. When i was a teenager i slept a lot and on the odd occasion i would not wake up until 4pm but I don’t think it does the body any good. Circulation for one thing cant be good if you sleep too long and poor circulation is not good for ones health. If your worried aobut it then maybe see a doctor?

Now you can see that sleep addiction is a sleep disorder, not necessarily a sleep illness, however, the most common people complain can be described as follows:

I must have sleep addiction. I simply cannot get out of bed to work out in the morning.

Let’s ask the doctor: (from Western Washington Univerity)

Q: Is there such thing as a “sleep addiction”? Because sometimes I think I have one.

I’ve often thought that if there were a “sleeping competition” at western I would win it by far. I also have this weird habit/ability to sleep almost anywhere, or anytime, no matter how much I’ve slept that day just because I like it that much. It was more of a problem for me last year, and I’ve limited my sleep and am doing fine, but sometimes – especially during midterms and finals – I can’t help it. Suggestions?

A: There are many reasons for “hypersomnia” which is a need/desire for excessive sleep–not exactly an addiction but roughly defined as sleeping more than 10 hours daily. There are a number of sleep disorders which can cause this, including narcolepsy, which can cause sudden sleep at inappropriate times (i.e. in class, while driving, during high stress situations) or sleep apnea which causes disrupted sleep patterns from hypoxia (low oxygen levels) so one is always sleepy, having not had restful sleep. There is depression which causes excessive sleep need, primarily due to fatigue, but also as an escape from the pain and worry of daily activities. Some medications and recreational substances can cause excessive sleep need. And then, a particular affliction of college students: no set sleep routine, no predictable hours of rest, being up at all hours of the night and so there is constant tiredness and sleep craving. Bottom line, if the excessive sleep need persists despite several days of at least 9 hours of sleep at night, then being evaluated in a sleep clinic for a sleep disorder makes a great deal of sense.

There are 24 time zones in the world, one for each hour in the day.

Each zone runs from North to South along Earth meridians that are approximately 1,000 miles (1,600 kilometers) wide.

Sometimes the actual width of each zone varies with geographical and political restrictions.

As the Earth rotates on its 24-hour schedule, sunrise occurs at a exact hour in one time zone, an hour later in the time zone immediately to the West, and so on through the 24-hour cycle.

Thus, in the USA when it is 5 a.m. in the Eastern Time Zone, it is 4 a.m. in the Central Time Zone, 3 a.m. in the Mountain Time Zone, and 2 a.m. in the Pacific Time Zone.sleep schedule

How jet lag occurs? The main reason for jet lag occurrence is failure of an air passenger to quickly adjust to the time in a different zone.

Thus, when someone who lives in San Francisco arrives in Rome at midnight Rome time, his or her body is still functioning on San Francisco time.

Another real problem is a changed in bathroom schedule causing constipation or even diarrhea.

If that happens, the brain may get confused and also disoriented by attempting to maintain the old schedules.

Sleep Paralysis has been considered as a very rare disorder.

Unfortunately, about 50% of all people now believe to encounter sleep paralysis sometime during their life.

Rapid eye movements (REM) sleep and slee paralysis

According to the latest study, sleep paralysis usually occurs when a person is moving into or out of REM (rapid eye movement) sleep.

More on REM Sleep see in this article. During REM sleep the body muscles are normally paralyzed.

During sleep, the brain is makes the body paralyzed until mind and body are synchronized.

It becomes clear now that Sleep Paralysis is the product of early mind-body detachment when a person is entering or exiting from REM sleep.

It has been noticed that Sleep Paralysis strikes when normal REM patterns is interrupted, especially after jet lag or periods of sleeplessness.

How sleep paralysis affect us

It has been also observed that Sleep Paralysis affects men and women equally of all ages, however, it is most common in teenagers.

Sleep Paralysis can be familial and may be genetic (inherited) in some cases.

Sleep Paralysis attacks are mostly harmless and short.

However, people who have been stricken by sleep paralysis, keep the memory of those terrifying sensations during sleep paralysis when they felt suffering. (Some scholars believe that sleep paralysis may account for some of the old claims of attacks by witches and the more recent “reports” of nocturnal abduction by space aliens.)

From a medical point of view, sleep paralysis is sometimes called waking paralysis (same as sleep paralysis), predormital (before-sleep) paralysis, postdormital (after-sleep) paralysis, and REM sleep atonia.

Good examples of such situations are:

Depending on their lifestyle, some people may need less than 6 hours of sleep while the others required more than 9 hours each night. How much sleep you need is also a factor of your age. Medical researchers and practitioners advice that every person between 16 to 65 years of age should get 7 to 9 hours of sleep per night. As a general rule, you are getting the right amount of sleep in the morning if you are able to wake up without the need of an alarm clock feeling restful and refreshed.

It happens when the upper airway becomes blocked during sleep. Most frequently, the blockage of airways happens when the soft tissue in the back of the throat collapses and closes the throat during the night. Another blockage happens when throat muscles are relaxed, and there is only a narrow airway in place. At this time, a large tongue or abnormal tissue in the throat can also block the airway.

Central apnea and mixed apnea are other types of sleep apnea, but are uncommon. In case of central sleep apnea there is a part of the brain, which controls our breathing mechanism, doesn’t work appropriately.

In addition, you could feel abnormally tired and sleepy during the working hours due to your sleep being disrupted throughout the night. You could even fall asleep at the wrong time of the day, for example, while at work or while driving. Sleep apnea is common sleep disorder.
